Fees and Dues: The total membership fee consists of 1) Enrollment Fee and 2) Monthly Dues.

I acknowledge that I know and understand the inherent risks of using this golf club practice facility and equipment. I know that these risks may include, but are not limited to, minor scrapes, strains and bruises, as well as significant injuries such as broken bones, eye injury or loss, concussion, paralysis and even death. By the execution of this agreement, I fully assume inherent risks associated with the use of this golf club and assert that I am voluntarily participating in such activities and hereby release Par Breakers Golf Academy and it's owners, employees and agents from liability for injury, death or property loss suffered by me resulting from ordinary negligence.

By member signature below, member agrees to the terms and conditions of membership, acknowledges receipt of a fully completed copy of this agreement and the rules and regulations of the club.

Rules & Regulations

•  Member's health warranty . Member warrants to the club that he/she has no disability, impairment or ailment preventing him/her from engaging in active or passive golf related exercise or that will be detrimental to his/her health, safety or physical condition if he/she does so participate.

•  Rules and regulations. Member agrees to abide by all the membership rules and regulations of the club which may be posted at the club, issued orally and/or published and which may be amended from time to time, at management's sole discretion.

•  Suspension/termination of membership by management. Management has the right to suspend and/or terminate any membership for non-payment of any dues or fees or for behavior that adversely affects the enjoyment of the club by other members, or for any other reason deemed sufficient. These decisions are based on the sole discretion of the management.

•  Inactive membership status . Memberships run continuous, and no temporary suspension is allowed because of travel schedules or personal need, except in the event of a work-related temporary relocation or illness.

•  Buyers right to cancel . During the initial agreement term, membership cancellation is only permitted if the member relocates more than 25 miles from its present location. In such a case, the membership dues will be converted to the month-to-month rate of $100 (if months Nov, Dec, Jan, Feb March) or $75 (if months April, May, June, July, Aug, Sept, Oct) . Cancellation is also granted in the event of disability or death. This member also may cancel if the club relocates more than 8 miles from its present location, or if services by the club are materially impaired. After the minimum agreement term, the member may cancel for any reason with 30 days notice of termination.

•  Entire agreement. This written agreement constitutes the entire agreement and understanding between the parties and is binding on the parties.
